Community Support Worker – Younger Adults North

Salary: £18,067 (Pro Rata)

 

As a Community Support Worker you will act as a key worker for a set amount of individuals, assisting with the provision of positive intervention and support for adults with mental health problems, physical and/or learning disabilities. Within this role your help and support will be aimed to help promote and encourage independent and community living. Personal care also included.

 

Responsibilities:

  • Key worker to a set number of customer’s. Supporting with hygiene routines and medication management
  • To support adults with mental health, physical and/or learning disabilities to encourage and promote independence
  • You will act as the first point of contact with Home Carers, providing advice in regards to any query that arises
  • Managing customer care plans. Making sure they are accurate, correct & up to date
  • To travel freely over Northamptonshire

  • Good IT skills including word and excel

 

Location: Da Vinci Court, Wellingborough

Rota to include: Early shifts, late shifts and alternate weekends
