Now is the time to join our fantastic, ever developing Reablement Team. We are seeking individuals who are passionate, resourceful and have limitless creativity.

 

As a Reablement Worker you will support frail or vulnerable adults, who may have had a change to their health, routine or lifestyle and are finding it difficult to live independently.

Within this role you will see first-hand the progression of your work and feel the difference that you are making to that person.

 

A full driving license and access to your own vehicle is essential for this role as you will be travelling countywide, supporting our customers within their own homes.

 

Main responsibilities:

  • Respond to emergency situations in the community in partnership with health services, assessing who can be supported at home and providing short term support to maintain the person’s safety.
  • Undertake risk, manual handling and medication assessments in order to safeguard both staff and person.
  • Maintain case records. Including written, verbal and using our bespoke IT system.
  • Liaise with health care professionals. Maintaining good links with local communities and other agencies.
  • Assess for, request and install low and mid-level equipment
