Reablement Support Worker – START South

Salary: £18,067 (Pro Rata)

 

Our Reablement services support adults, who may have had a change to their health, routine or lifestyle and are finding it difficult to live independently. We support frail and vulnerable adults in their own homes following a stay in hospital or alternative to prevent them from being admitted to hospital. Our short-term care team helps people to remain living at home safely and independently.

 

Responsibilities:

  • Respond to emergency situations in the community in partnership with health services, assessing who can be supported at home and providing short term support to maintain the person’s safety.
  • Undertake risk, manual handling and medication assessments in order to safeguard both staff and customer.
  • Maintain case records. Including written, verbal and using our bespoke IT system.
  • Liaise with health care professionals. Maintaining good links with local communities and other agencies.
  • Assess for, request and install low and mid-level equipment.

 

 

Abilities & Skills:

  • Good written and communication skills. Ability to write clear and concise reports, care plans and any other additional written statements.
  • Have the ability to listen and interact accordingly with the customer and on the customer’s behalf to any health care professional.
  • To work under pressure and to work to time frames.
  • Flexible, adaptable and tolerant to meet the needs of the service
  • Kind, compassionate and caring.

 

Location: Chedworth Close, Ecton Brook, Northampton

Rota to include: Early shifts, late shifts and alternate weekends.
