Shared Lives Officer

We are looking for a highly motivated, enthusiastic individual with a ‘can do’ attitude, who can work independently and as part of the team, to support a range of carers and customers across the county to maintain CQC compliance support independence, and promote choice.

 

Working with external professionals and advocates to achieve the best outcome for individuals.

 

You will effectively manage a caseload of Shared Lives Carers and offer support and advice as appropriate ensuring that carers have sufficient training and resources to undertake their roles effectively and meet all of the needs of the customer in placement.

 

Carry out regular monitoring and support visits, carry out biannual medical and financial audits and hold Shared Lives Scheme Carer and Customer Reviews to assess support levels, outcomes, training needs, and the setting of objectives for the next year.

 

A full driving licence is essential.
