Our Crisis Response Team is a fast paced, countywide service, that supports adults with social care needs who are either being discharged home from hospital, or who are in a crisis situation and at high risk of being admitted to hospital due to social care needs. The team are responsible for conducting social care assessments and providing hands on care and support to customers for a short period of time. This can include supporting a customer with management of their medication, personal care tasks, provision of food and drink and other essential daily living tasks.
As a supervisor you will have excellent leadership and co-ordination skills, and will also have a good understanding of Care Act legislation and CQC Key Lines of Enquiry. You will be responsible for supporting, supervising and appraising a group of Reablement Support Workers. You will also be required to carry out a shift lead role, taking on the responsibility of the day to day running of the service in the absence of the team manager or team leader of the service.
